Notice of Privacy Practices.

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment

Tyler McEwen, LCSW ("the practice") is required by the Health Insurance Portability and Accountability Act (HIPAA) and California law to protect the privacy of your protected health information (PHI), provide you with this notice of legal duties and privacy practices, and follow the terms of the notice currently in effect.

How we may use and disclose your information

• Treatment. To provide, coordinate, or manage your mental health care, including consultation with other providers when clinically appropriate and authorized by you.
• Payment. To bill and collect payment for services, including providing a superbill or statement that you may submit to your insurance.
• Health care operations. For activities necessary to operate the practice, such as quality review and administrative tasks.
• Required by law. When disclosure is required by federal, state, or local law (for example, mandated reporting of suspected child, elder, or dependent adult abuse).
• Serious threats to safety. To prevent a serious threat to the health or safety of you or others, consistent with applicable law.

Your rights

• Request to inspect and obtain a copy of your record.
• Request an amendment to your record.
• Request an accounting of certain disclosures of your information.
• Request restrictions on certain uses and disclosures (which the practice will consider but is not always required to grant).
• Request confidential communications by alternative means or at an alternative location.
• Receive a paper copy of this notice upon request.
• File a complaint if you believe your privacy rights have been violated, without retaliation.

Uses requiring your written authorization

Most uses and disclosures of psychotherapy notes, uses for marketing, and sales of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent the practice has already acted on it.

Changes to this notice

The practice reserves the right to change this notice and to make the new provisions effective for all PHI it maintains. The current notice will be available upon request and posted at this URL.
